What Nobody Told You About Cholesterol and Menopause

Let's start with some statistics that still don't get said enough. 1 in 3 women will die from cardiovascular disease. It kills more women than all cancers combined, and approximately 1 woman dies from it every 80 seconds in this country. Nearly 45% of women over 20 already have some form of it.


I don’t mean to scare everyone, but this is so important to know. One of the biggest changes during menopause is often one you can't see or feel and that is that is that your cardiovascular risk begins to change. Heart disease is actually up to 80% preventable. The 2026 Dyslipidemia Guidelines, just released this last spring by the American College of Cardiology and the American Heart Association, give us better tools to find risk early and act before damage is done. 


Why the guidelines changed for cholesterol, and why it matters

We have learned that heart disease begins years or even decades before someone has a heart attack or develops symptoms. For years, cholesterol care focused mostly on LDL, but we've also learned that looking at LDL cholesterol alone doesn't always tell the whole story.  We now understand that triglycerides, ApoB, and Lp(a) all tell us important things about cardiovascular risk, and many people have never been tested for them.


This is a great shift as we can now identify risk earlier and do something about it. Instead of waiting until plaque has already built up, we have the opportunity to focus on prevention much sooner. We also have more treatment options, with new therapies under research and continuing to emerge. 


Why menopause is a cardiovascular turning point, not just hormonal

When women come to see me, they are often looking for help with hot flashes, poor sleep, weight changes, brain fog, or low energy. Those things matter greatly, but menopause is not just about symptoms. It is one of the most important times in a woman's life to think about long-term health.


Before menopause, estrogen helps support healthy blood vessels, influences how your body processes cholesterol, and helps your liver clear LDL more efficiently. As estrogen declines, LDL and triglycerides often rise, body fat tends to shift toward the abdomen, insulin resistance becomes more common, and blood pressure may begin to climb. After menopause, a woman's cardiovascular risk rises significantly, with the incidence of heart attack increasing over the following decade.


None of this is inevitable, but it does mean that the menopause transition is an important window of opportunity to look at cardiovascular health and lower future risk before disease ever develops. 


When I am evaluating someone during perimenopause or menopause, I am not only asking how she feels today. I am thinking about where she will be 10, 20, or even 30 years from now. That means looking at cholesterol, blood pressure, blood sugar, body composition, family history, exercise habits, sleep, and inflammation. Helping women feel better is incredibly important. Helping them reduce their future risk of heart disease, stroke, and cognitive decline is just as important. 


I do not separate heart health from brain health. The same plaque-building process that affects your heart arteries can also affect the arteries feeding your brain. That means when your lipid levels shift during this transition, we are not just talking about heart attack risk. We are talking about stroke risk too. 


Your health history matters more than you might think

The 2026 guidelines formally recognize something called risk enhancers which give meaningful clues about your future heart risk.


  • A history of gestational diabetes, preeclampsia, or eclampsia during pregnancy. Pregnancy is basically a stress test on our bodies! 

  • Premature menopause, meaning before age 40

  • Autoimmune conditions like psoriasis or rheumatoid arthritis


Even if these things happened 20 or 30 years ago, they are still important. 


What your lipid panel is actually telling you

A standard lipid panel needs a 12 hour fast to be accurate, since eating can temporarily raise triglycerides.

  • LDL (a cholesterol particle that can build up in artery walls): tends to rise during the menopause transition. This matters for both your heart arteries and the larger arteries linked to stroke risk.

  • HDL (a cholesterol particle that helps carry excess cholesterol away from your arteries): often stays normal or even goes up during this transition, but a higher number does not automatically mean better protection.

  • Triglycerides: often rise too, especially with insulin resistance, diet, and alcohol intake.


Why your LDL is probably creeping up even if nothing else has changed

Why does this happen? Your liver uses specific receptors on its cells to pull LDL out of your bloodstream. Estrogen appears to keep those receptors working efficiently. As estrogen declines, that process seems to slow down, and LDL starts to accumulate instead of being cleared. On top of that, estrogen also appears to shift your body toward producing less of the protein that builds LDL and more of the protein that builds HDL. 


This is the leading explanation based on current research, alongside diet, activity, and genetics, which all still play a role. There is also a concept I want you to know called LDL-years. Think of it like pack-years for smoking. What matters is not just how high your LDL is right now. It is how many years your body has been exposed to elevated levels. The earlier we get things under control, the more protection you build over time. 


Your new risk calculator: the PREVENT score

The 2026 guidelines replaced the old cardiovascular risk calculator with something much better called the PREVENT score. The old tool was built on data from about 26,000 people. The PREVENT score was developed using data from 6.6 million people, making it far more accurate for the population we are actually seeing today.


What I find particularly useful for my patients: for women aged 30 to 59, it now calculates your risk over the next 30 years, not just the next 10. That long view matters enormously during the menopause transition, because the decisions we make right now have consequences that extend decades into the future.


The risk categories under the new framework look like this:

  • Low risk: Keep doing what you are doing

  • Borderline risk: Time to look carefully at lifestyle

  • Intermediate risk: Medication may be worth a real conversation

  • High risk: We need to act


What LDL number are we actually working toward?

The 2026 guidelines bring back specific LDL targets based on your individual risk level. This is called a treat-to-target approach. Instead of starting medication and hoping for the best, we now have an actual number to work toward together.


This can be surprising, but a very low LDL is not something to be afraid of. When we are born, our LDL levels are naturally around 30 mg/dL. Targets like 55 or 70 mg/dL are not extreme. They reflect what our bodies were actually designed to handle.


Here are the current targets:

  • General or low risk: under 100 mg/dL

  • High risk: under 70 mg/dL

  • Very high risk: under 55 mg/dL


The test that goes deeper than LDL: ApoB

A standard lipid panel measures how much cholesterol is sitting inside your LDL particles. What actually builds plaque in your arteries is the number of particles, not how much cholesterol each one is carrying. 


ApoB is a blood test that tells us how many cholesterol particles are actually circulating in your blood. This matters because each of those particles has the potential to enter the artery wall and contribute to plaque buildup. In many cases, knowing the number of particles gives us a better picture of your heart disease risk than looking at LDL cholesterol alone.


The new guidelines recognize ApoB as an important tool, especially when your LDL looks normal but your risk may still be higher than it appears. For example, your LDL cholesterol can be in a good range, but if your ApoB is elevated, you may still have a large number of cholesterol particles that can contribute to plaque over time.


ApoB can be especially helpful for people with diabetes, high triglycerides, or existing heart disease, but it can also provide valuable information for others as well. The test is simple, doesn't require fasting, and can usually be added to your regular blood work. The only catch is that it isn't included in a standard cholesterol panel, so you usually have to ask for it.


The genetic marker many women have never had tested: Lp(a)

Lp(a), which we pronounce "L-P-little-a," is a particle that is structurally similar to LDL but has an extra protein attached that makes it more prone to promoting both plaque buildup and blood clotting. This is part of why it raises the risk of both heart attack and stroke specifically.


In a large study following more than 126,000 people over time, higher Lp(a) levels were linked to increased risk of both the most common type of stroke and the less common bleeding type. This is not just a heart marker. It affects the same vascular system that feeds your brain.


The 2026 guidelines now formally recommend that Lp(a) be measured at least once in adulthood for everyone. Here is what the numbers mean in practice:

  • A level at or above 125 nmol/L increases your cardiovascular risk by about 40%

  • A level at or above 250 nmol/L approximately doubles your long-term risk


Here is what makes Lp(a) different from everything else on your panel: it is almost entirely genetic. Your diet, your exercise routine, your weight, none of these move it much. That can feel frustrating to hear, but it is actually important information. It tells us to be more aggressive about managing everything else that is in your control.


One nuance: Lp(a) does appear to rise somewhat during the menopause transition itself, possibly related to declining estrogen, and separate from your genetic baseline. So if you were tested years before menopause, it may be worth asking about a recheck, especially if you have other risk factors or a family history of early heart disease or stroke.


Right now there is no widely available medication specifically designed to lower Lp(a), but this is one of the most active areas in cardiovascular medicine. Several drugs in large clinical trials have shown early reductions of up to 94% or more. We are waiting for more data that can tell us whether lowering that number actually translates into fewer heart attacks and strokes.  


The coronary calcium scan: a useful tie-breaker

If you and your provider are on the fence about whether to start cholesterol medication, the 2026 guidelines support using a coronary artery calcium scan as a tie-breaker. This scan looks for actual calcium deposits inside your heart arteries, which are a direct sign of plaque that has already formed. It does not show the soft plaque. If calcium is present, the case for medication becomes much stronger. If the score is zero with no other high-risk conditions, it may be reasonable to hold off and recheck in a few years.


One tip I share with patients: if you have ever had a chest CT for any reason, like a lung screening or follow-up after an illness, ask to see that report and look for any mention of calcium. This information is often noted incidentally and helpful to know.


How you take hormone therapy matters for your lipids

The route of estrogen matters, and this is something I talk about with patients regularly.

Oral estrogen passes through the liver first before entering your bloodstream. This can lower LDL, but it can also raise triglycerides for some women. Transdermal estrogen, meaning patches or gels absorbed through the skin, bypasses that first pass through the liver and tends to have a more favorable effect on triglycerides overall. For women who already have high triglycerides or elevated cardiovascular risk, transdermal is often the preferred route for that reason.


There is also the timing hypothesis. Research suggests that starting hormone therapy within about 10 years of your last period, or before age 60, is associated with better outcomes for both heart and brain vascular health than starting later. This is not a hard rule that applies identically to every woman. It is a meaningful consideration that deserves a real conversation with your provider, especially if you have a personal or family history of stroke.


What you can actually do about all of this


Start with lifestyle, because it still matters most

Cutting back on refined carbohydrates and added sugar and replacing them with fiber-rich foods and quality protein is one of the most effective things you can do for both LDL and triglycerides. Saturated fat still matters for LDL too. Both cardio and resistance training support your lipid levels, with strength training being especially important during this transition for reasons that go well beyond cholesterol.


Aim for at least 5,000 to 10,000 steps a day as a starting point. And please do not underestimate sleep. The American Heart Association now formally includes sleep as part of its Life's Essential 8, on equal footing with diet and exercise. If perimenopause is disrupting your sleep, which it does for so many women I see, that is worth addressing as part of your cardiovascular health plan, not just as a quality of life issue.


Lifestyle and medication are not alternatives to each other. They work together, and both matter.


When lifestyle is not enough: what your medication options look like now

Statins are still the cornerstone of LDL treatment per the 2026 guidelines, and the evidence behind them is strong. However, they are not your only option and if you have tried one before and were not able to tolerate it for one reason or another, that is not the end of the road. The 2026 guidelines formally address this for the first time with evidence-backed alternatives.


When a statin alone is not enough or not tolerated, the next step is typically adding an oral medication. Ezetimibe (Zetia, available since 2002) reduces how much cholesterol your intestines absorb and is generic, affordable, and well-tolerated. Bempedoic acid (Nexletol, approved 2020) works similarly to statins but only activates in the liver, not in muscle tissue, which is exactly why it avoids the muscle symptoms that push so many people off statins.


The two are also available together as a single combination pill as well. The 2026 guidelines gave bempedoic acid multiple Class 1 recommendations specifically for statin-intolerant patients, and together these two oral options can achieve meaningful LDL reductions without a statin at all.


For patients who still have not reached their LDL goal on oral medications, there are injectable options including PCSK9 inhibitors and inclisiran, a newer injectable that works differently and requires less frequent dosing. These can be highly effective, but: they are expensive, insurance coverage can be difficult to navigate, and access is not always straightforward. If one of these comes up in your care, it is worth a detailed conversation with your provider about what is realistic for your situation.


I also want you to know that this area of medicine is changing quickly. We are learning more every year, and new treatments are being developed all the time. The options we have a few years from now will likely be very different from what we have today.


That is exciting because it gives us more ways to help prevent heart disease and treat it earlier. It is also why it is so important to stay informed and continue having conversations with your healthcare provider. The more we learn, the more we can personalize care to your individual risk and help you stay healthy for years to come.


A simple framework to remember: the AB CS

The 2026 guidelines offer a framework I like sharing with patients because it makes a big topic manageable:

  • A for Assessment: Know your PREVENT score and discuss whether aspirin is appropriate for your situation

  • B for Blood Pressure: Aim for less than 120/80

  • C for Cholesterol and Cigarettes: Work toward your LDL target and avoid smoking or vaping

  • D for Diet and Diabetes: Manage blood sugar and eat in a way that supports your heart

  • E for Exercise: Aim for 5,000 to 10,000 steps daily and include resistance training


The bottom line

The menopause transition brings real, measurable changes to your lipid levels, and those changes affect your whole vascular system, not just your heart. The 2026 guidelines give us better tools than we have ever had to understand your individual risk, set a real target, and choose the right path to get there.


The core message is simple: lower for longer. The earlier you get your cholesterol to a healthy level, the more protection you build over time, and that protection compounds in ways that matter for your heart and your brain decades from now.
