Your Brain on Menopause: Why You Feel Different and What You Can Do About Brain Fog.

Have you ever walked into a room and completely forgotten why you went there? Have you been in the middle of a conversation and suddenly lost your train of thought?  Maybe you just can’t find the word you have been searching for and then it pops into your head an hour later?Women describe it differently, but I hear the same symptoms over and over in my office.


 "I feel like I'm in a fog.”


"My brain doesn't work the way it used to."


It's hard not to wonder if you're stressed, overwhelmed, getting older, or if something is actually wrong. Part of that fear comes from the fact that women make up nearly two-thirds of people living with Alzheimer's disease. We don't fully understand all the reasons why, but researchers are actively studying the roles of longevity, genetics, cardiovascular health, and the hormonal changes that occur during menopause.The good news is that what you're experiencing is a very common part of the menopause transition. 


Nobody Told Us Menopause Affects the Brain

If we were taught anything at all about menopause, most of us were taught that menopause is about our ovaries and our hormones. Nobody talks about the fact that menopause is also a brain transition.


As I have described in previous blogs, estrogen does so much more than regulate periods and hot flashes. It plays an important role in memory, mood, sleep, temperature regulation, and even how the brain uses energy. Estrogen receptors are found throughout the brain, so when estrogen begins fluctuating during perimenopause and eventually declines, it makes sense that the brain will be affected.


For years, women were told these symptoms were stress, aging, or something they needed to push through. While stress and poor sleep play a part, there is so much more to it. 


The Renovation Project Happening Inside Your Brain

One researcher whose work I find fascinating is Dr. Lisa Mosconi, a neuroscientist who studies women's brain health. She describes menopause as a renovation project on the brain.


I really like that analogy, because a renovation is not a house falling apart; it's a period of change. Things may feel messy while the work is happening, but that doesn't mean something is wrong.


Researchers have found measurable changes in the brain during the menopause transition, including changes in how the brain uses energy, blood flow, and brain structure. We're still learning exactly what these changes mean long term, but one thing is clear: the cognitive symptoms many women experience during this time are real.


What is actually happening in the brain during the menopause transition.


Brain fog and energy: One of estrogen’s most important jobs is helping brain cells use glucose efficiently. Glucose is basically the brain's fuel. When estrogen starts to decline, the brain has to figure out a new way to find and fufill the energy needs.

That can be shown on brain scans. Neuroimaging studies have shown a large drop in how much glucose the brain is taking up during the menopause transition, particularly in the areas of the brain that are responsible for memory and focus. The brain responds by trying to use other fuel sources to compensate.


Mood, anxiety, and irritability: This is one I feel strongly about because women have been told for so long that their mood changes during menopause are just stress, or aging, or something they need to push through, however the science shows us that there is actual biology behind it.


Estrogen receptors are found throughout the parts of the brain that handle how we feel and how we respond to stress. That includes the amygdala, the hippocampus, and the prefrontal cortex. Estrogen also directly influences serotonin, which most people know as the feel-good brain chemical. It helps the brain make serotonin and keeps those receptors working well. When estrogen drops, that system weakens.


What I find very interesting is a 2024 study with brain imaging that showed in live human brains, that higher estrogen receptor activity in the amygdala and frontal areas actually predicted whether women reported mood and cognitive symptoms after menopause. 


This does not mean hormones explain every difficult day, but it does explain why so many women describe feeling more irritable, more anxious, or emotionally vulnerable during this time.  


Hot flashes and the hypothalamus: Most people do not know what is actually happening during a hot flash. The hypothalamus is the part of the brain that acts like your body's internal thermostat. Inside the hypothalamus, there is a specific group of neurons called KNDy neurons. During your reproductive years, estrogen keeps these neurons calm and regulated. When estrogen falls at menopause, that calming effect disappears and these neurons basically start to go into overdrive. When that happens, they release a surge of a signaling molecule called neurokinin B. That signal affects the nearby temperature control center and triggers the body's heat-release response, which is what we experience as a hot flash: the flushing, the sweating, the sudden wave of heat. This is what led to new treatment for non hormonal options for hot flashes. NK3 receptor antagonists, such as Fezolinetant (Veozah), work by blocking that neurokinin B signal. 


Brain resilience and adaptation: The brain does not just give up when estrogen changes. It adapts, but Imaging studies show us that the brain is working hard to compensate. We see changes in glucose metabolism, in blood flow, and even in brain structure during the menopause transition. Blood flow and glucose uptake, which normally move together in a healthy brain, can become uncoupled during menopause. A 2024 imaging study also found that estrogen receptor density in key memory and cognitive networks increases during the menopause transition. The interpretation is that the brain is turning up its sensitivity since there  is less estrogen available. It is trying to capture every bit of signal it can.


Is It Brain Fog or Something More Serious?

A fearful question many women ask is, "How do I know if this is menopause or dementia?"

I heard this in a podcast with Dr Lisa Mosconi and Dr Mary Claire Haver. A simple way to think about it is this: Forgetting where you put your keys can happen with brain fog. Forgetting what keys are used for is more concerning.

Forgetting a name and remembering it later is common. Getting lost in familiar places, struggling to manage finances, or noticing symptoms that steadily worsen over time need further evaluation. Most menopausal brain fog is not dementia, however, that doesn't mean women should be dismissed. If your symptoms are affecting your quality of life, they deserve attention.


Why Sleep Matters So Much for the Brain

Sleep is one of the most important things we can do for our brains. During sleep, the brain is busy regulating hormones, consolidating memories, and performing important maintenance and repair functions. Researchers have also identified a waste clearance system in the brain called the glymphatic system, which appears to be most active during sleep.

Unfortunately, sleep is often one of the first things disrupted during perimenopause. Hot flashes, night sweats, anxiety, and those frustrating 3 a.m. wake-ups can leave women exhausted. When sleep suffers, everything else tends to suffer too. Brain fog feels worse. Focus becomes harder. Mood becomes more fragile. That is the one reason I often start with sleep.


Menopause Is a Window of Opportunity

One of the most important things I've learned from both menopause research and brain health research is that midlife is a critical window for long-term health and brain health. This is often when blood pressure starts creeping up. Cholesterol changes. Muscle mass declines. Sleep worsens. Blood sugar becomes more difficult to manage. Estrogen begins to fluctuate.


The good news is that there are steps we can take to combat it.


What's Good for Your Heart Is Good for Your Brain

One of the things I talk about all the time in my practice is cardiometabolic health.

The brain depends on an incredible network of blood vessels to deliver oxygen and nutrients. When we take care of our cardiovascular health, we're also supporting our cognitive health. That's one reason I pay attention to cholesterol, ApoB, insulin resistance, body composition, sleep quality, and physical activity.

We're not just thinking about how you feel today. We are thinking about our 80-90 year old selves.


Don't Ignore Your Hearing and Vision

Recent research has identified hearing loss and vision loss as potentially modifiable risk factors for dementia. If you've noticed changes in your hearing or your vision, don't brush them off as "just aging."


We need to make sure we are getting our vision and our hearing checked. We need to wear glasses or contact lenses if we need them and wear hearing aids if we need them!


The Four-Legged Stool of Brain Health

I recently heard brain health experts describe brain health as a stool with four legs, and I thought it was a helpful image.


The seat of the stool is built on two foundations: movement and nutrition. Walking, strength training , cardiovascular exercise, vegetables, beans, fish, olive oil, nuts, whole foods. Not at all at once, but one change at a time and staying consistent. 


The other legs of the stool are sleep, stress management, social connection, and preventive healthcare. If one leg becomes weak, the stool becomes less stable. We often spend so much time searching for what we think is going to be the answe as a quick fix. However, brain health comes back to the basics done consistently.


Let's Talk About Hormone Therapy

As I stated above, estrogen receptors are found throughout the brain, which helps explain why so many women notice changes in how they think, feel, and function during the menopause transition.


Hormone therapy remains the most effective treatment we have for hot flashes and night sweats. When women are sleeping better, having fewer hot flashes, and feeling more like themselves again, it's not hard to see the connection between hormones and brain health. However, we can't say that hormone therapy is currently recommended solely for the prevention of dementia or cognitive decline. The research is exploring the relationship between hormones like estrogen and how they influence long term brain health. I am excited to see what the research shows.


If You Don't Feel Like Yourself, There Is a Reason

Nobody warned us that menopause could affect the brain. We know estrogen plays an important role in the brain. We know the cognitive symptoms many women experience during perimenopause are real. We know that sleep, exercise, nutrition, muscle, cardiometabolic health, social connection, hearing, and vision all matter. We know that women deserve better answers than, "That's just aging."


What You Can Do 

If all of this feels overwhelming, start here.


Think of these as actionable items to support your brain during midlife. These are goals. Aim for small additions/small tweaks in what you are doing already.


Prioritize sleep and address hot flashes, night sweats, snoring, or possible sleep apnea

Strength train at least 2 to 3 times per week to maintain muscle, metabolic health, and independence.


Move your body most days, whether that's walking, cycling, swimming, dancing, or something else you enjoy


Eat a Mediterranean eating pattern with plenty of vegetables, beans, nuts, olive oil, fish, and minimally processed foods


Get enough protein to support healthy muscle mass as you age


Know your numbers: blood pressure, cholesterol, ApoB, blood sugar, and insulin resistance


Stay socially connected and maintain meaningful relationships


Manage stress in ways that work for you, whether that's exercise, yoga, meditation, therapy, or time outdoors.


Have your hearing checked if you find yourself asking people to repeat themselves


Have regular eye exams and wear the glasses or contacts you need


Talk with a qualified healthcare professional if symptoms are affecting your quality of life


The goal isn't perfection. The goal is progress.
